Visa Clearance Form
Only International students who have recently attended a U.S. institution (this includes both high school and college/university in the U.S.) need to fill out this form.
For proper verification of immigration status, the Visa Clearance Form [.pdf - 72kb] should be filled out, signed, and mailed to FGCU by your Designated School Officer from the previous institution.
The form is required before we can make an admissions decision, so please make sure to have this form completed at the time of your application for admission to FGCU. Failure to provide this form will delay the application process.
