Adding, Moving, Changing, or Removing Phones 

Service requests are for planned changes and include, but are not limited to, the following: Click here for rate information.
  • A new installation
  • A disconnect
  • Changes to telephone set features and configurations
  • Changes to the model of a telephone set
  • Additions or moves of existing telephones
  • New voice mailboxes
  • Changes to voice mailboxes
  • Directory name changes

All requests for service related to the University phone system are made by completing a Telecommunications Service Request form (TSRF) which can be accessed online here:

Expected completion time is 5-7 business days. However, the Department of Telecommunications would appreciate as much notice as possible.

A monthly and/or one-time charge may apply for requested service. Please contact the Department of Telecommunications for further pricing information.
