Academic Retention Program


Guide to Forms

A) Here are the forms you will need to fill out before your initial meeting with us:

Personal Action Plan - This form outlines your choices for assistance for the semester.

Academic Probation Policy - Outlines the FGCU academic standards policy per the student catalog. You must sign and date this form.

Student Information - Gives us basics information about our students.

Student Questionnaire - This form pinpoints specific behaviors that may be responsible for your present status.

Inventory of Common Concerns - A tool to help you focus on various academic issues.

B) Optional Forms:

Academic Retention Program Update - Complete each section every other week and submit to CAA electronically by clicking the submit button.

List the grades you have received in all of your classes thus far (homework, tests, reports, projects, etc...). This helps keep track of your academic progress as the semester proceeds.

Indicate whether you would like some tutoring in a course. You may also fill out an assistance request form. If you are receiving tutoring, please list course name, tutor name and times of tutoring. Please indicate how the tutoring sessions are going.

Indicate whether you able to keep up with your FGCU study schedule. If not, why not.

Identify any courses you are concerned with. We highly encourage you to speak with your professor(s) regarding your concerns. and stress management. Track the progress of these personal ratings throughout the semester.

Study Schedule - Determine the number of study hours you need to achieve your goal grade in each class. (At least 1-3 hours of study per credit hour must be allocated, depending upon the difficulty of the course material. Some courses, such as College Algebra or Statistics, may require more study hours.)

Plan and total the number of study hours per week. Set up a study schedule based upon the total hours calculated per week. Mark the days and times you are in class, at work, or committed to other things. Then determine which of the remaining days and times are best for you to study. We recommend studying at the library at least half of the time.

Academic Retention Program Evaluation Complete each section and submit to CAA  electronically by clicking the submit button at the end of term.
