Florida Gulf Coast University (FGCU) enters into various contracts with numerous outside organizations and individuals providing services.  Under the terms and conditions of a contract or agreement for services, the outside entity (contractor, consultant, or vendor) must show evidence of adequate insurance coverage by submitting a Certificate of Liability Insurance (COLI). This certificate must exhibit minimum University limits of liability requirements and an endorsement naming FGCU as additional insured for general liability.

The COLI is sent to the FGCU department seeking services and a copy must also be forwarded to the FGCU EHS department. Click HERE for the matrix showing minimum insurance requirements for those that seek to do business with FGCU. The university may permit exceptions to these requirements. These minimum insurance limits requirements do not apply to university construction projects.

The certificate(s) is required to contain the following insurance information:

• Name Florida Gulf Coast University Board of Trustees, the university and the officers, employees, agents, and volunteers as additional insured, except for professional liability and workers’ compensation insurance.
• The certificate and policy shall indicate that coverage’s afforded under the policy will not be cancelled, modified, changed, or expire before 30 days prior written notice has been given to the University.
• The insurance underwriter must be rated as “A” or better by A.M. Best Company.
