How to Acquire Camp Insurance
General overview
Camp insurance is available to University departments having day or overnight sport and educational camps.
This camp insurance is a supplemental insurance program is offered through state contract insurance vendor ACE American Insurance Company. It is designed for participants in sport and educational camps in need of primary or excess insurance coverage for injuries and/or illness sustained during the camp - on or off campus.
Responsibilities
Camp Sponsor has the primary responsibility of applying and paying for coverage as well as contacting the Assistant Director of EHS within 24 hours of accident or illness. Camp Sponsor must ensure that medical release forms are complete. Camp Sponsor must immediately notify EHS of updates to camp roster.
Environmental Health and Safety has the primary responsibility of offering this supplemental insurance for sport and educational camps held by FGCU.
Procedures for obtaining coverage for camps

Review plan options and submit completed application and first draft of roster with the Assistant Director of Environmental Health and Safety (EHS), Willie Baca, on or before the 28th of the month prior to the camp start date. For example if a camp will started at anytime in July, the application and roster is due on or before June 28.

Send updated roster spreadsheets to EHS immediately every time there is a change of participants.

Ensure medical release forms are complete and signed by parent or legal guardian of camper. Medical release forms are to be kept secure by the coach, faculty, or staff person that is responsible for camp.

Pay invoice. Note: A change in the roster will result in a refund for overpayment or an additional invoice for underpayment.

Plan Options Suggested:
Primary - Plan 1
Purchasing this insurance will cover participants 100% for camp related injuries, other than policy-excluded items.
$0.25 for day or overnight camps
$0.45 for sport camps
Primary with sickness - Plan 2
Purchasing this insurance will cover participants 100% for camp related injuries and illness with onset during the camp schedule, unless it is an exclusion item listed in policy. Sickness begins when a physican first diagnoses and recommends treatment for it.
$0.60 for day or overnight camps
$0.95 for sport camps
