This incident report form shall be used when a student,  visitor, or off-duty employee sustains an injury here on campus. Submit form within 24 hours.	Non Work Related Injury When a student or visitor gets hurts anywhere on FGCU property and sustains a serious injury were EMS or Campus Police are called to assist, please fill out this form and submit within 24 hours.  Incident Report for non-employees
