Replacement Cards
Replacement Fees:
There is a $15.00 replacement fee for lost, stolen or damaged Eagle ID Cards.
To report a lost or stolen card
- Call 239-590-7645 during normal business hours (Mon-Fri, 8am-5pm)
- Call 239-590-1900 (FGCU PD) before or after normal business hours.
No Replacement Fees:
Mag Stripe Malfunction
A "malfunctioning" card is issued at NO charge provided that the card meets the requirements listed below.
- The black Mag strip on the back of the card has NO visible damage.
- The ID card itself has NO damage.
- The card is the current active card.
- The card cannot be read for all activities.
Name Change
A name change is issued at NO charge provided that the current active card is returned.
Stolen Cards
A stolen card is issued at NO charge provided that a detailed police report is submitted.
(A case number is not a police report.)
How to obtain a replacement ID Card:
On Campus
The Eagle ID Card Office now accepts cash and checks for ID card replacements. If you are paying for a replacement with a credit card or with Eagle Dollars, you must go to the Cashier's Office to pay the fee. Afterwards bring your receipt to the ID Card office to receive your replacement Card.
Off Campus
Distance learners must submit the replacement card form and the fee in the same manner that they did to obtain their original card, which is by completing the .
The fee can be submitted by:
- Mailing the fee with the form.
- Calling the Cashier's Office (239)590-1213 to find out their payment options.
