Lost, Stolen, or Damaged Card
There is a $15 replacement fee for lost, stolen or damaged Eagle ID cards. Cards that need to be replaced due to malfunction or name change are issued at no charge provided that the original card is returned when a replacement is requested. To report a lost or stolen card, call (239) 590-7645 during normal business hours, 8:00am to 5:00pm, Monday through Friday. Before and after normal business hours and on weekends, contact the FGCU Police Department at (239) 590-1900.
| HOW TO OBTAIN A REPLACEMENT CARD |
The Eagle ID Card Office now accepts cash and checks for ID card replacements. If you are paying for a replacement with a credit card or with Eagle Dollars, you must go to the Cashier's Office to pay the fee and then bring your receipt to the ID Card Office to receive your replacement card. Distance learners must submit the replacement card form and the fee in the same manner that they did to obtain their original card, which is by completing the User Request Form. Distance Learners will be required to disclose their University ID number on the replacement card form in order to verify their identity to University personnel. The replacement Eagle ID card will then be mailed to the cardholder.
