TRANSCRIPTS:  Please list all colleges and universities ever attended on your graduate application.  Official transcripts (in their originally sealed envelope) must be requested from all colleges and universities that comprise your entire academic record.

REQUIRED IMMUNIZATION FORM:  All new applicants must submit this form along with proper documentation.  If you are a previous FGCU student, and/or are currently enrolled at FGCU, these forms are already on file and don't need to be resubmitted.  CLICK HERE

SSN Statement:  Federal Law allows the university to require that you provide your U.S. Social Security Number for the purpose of identification and verification of student records, including, registration, financial aid, and academic records (such as GMAT, GRE, or MAT scores and transcripts), verification of identity in connection with the provision of university services, and for required state and federal reporting as well as institutional purposes.  By providing your SSN, you authorize the University to disclose it to third parties as necessary for these purposes.

Users who have forgotten their PIN must request it in person at the Office of the Registrar or Admissions with a photo ID; or through the mail by sending a signed letter with signature, birthdate, and Social Security Number to the Office of Graduate Studies, 10501 FGCU Blvd. S., Fort Myers, FL  33965.
