We are excited that you have chosen to apply to FGCU Graduate Studies!  Please find applications and required forms to apply below. 
IMPORTANT:  Official, hard copy transcripts (in the original sealed envelope) from every college and/or university that comprises your entire academic record are required unless you have graduated from FGCU or currently attend FGCU for your bachelor’s degree.  You may submit unofficial transcripts in order to expediate the processing of your file, but if admitted to the program, you will need the official transcripts before you register for classes. 

Please note: 
When requesting college transcripts, make sure to request a hard copy to be mailed to our office as many institutions will send electronic transcripts automatically and we are not able to accept them.

A graduate entrance exam is required and some programs also require supplemental materials to be submitted.  Please visit our Admission Requirements & Deadline Dates page to view the requirements for your specific program..

APPLICATION FEE:  To submit an application online, a $30.00 non-refundable application fee in US dollars is required and can be paid online using your VISA, AMERICAN EXPRESS, DISCOVER or MASTERCARD.  If you choose to complete the printable version of the application, please submit a $30.00 check payable to FGCU.

RETURNING FGCU STUDENTSIf you have previously attended FGCU, and/or are currently enrolled at FGCU, you are exempt from the $30.00 application fee.  Please click on the Online Graduate Application link above and follow the steps for Returning FGCU Students.

DISTANCE LEARNINGProspective students resident in the following states (Arizona, California, Connecticut, Delaware, Hawaii, Illinois, Indiana, Michigan, Mississippi, Montana, Nebraska, New Hampshire, New Jersey, New York, Rhode Island, South Carolina, South Dakota, Vermont, Virginia, and Wisconsin) that wish to pursue a graduate degree or certificate program entirely online within their state of residence may submit an application according to university procedures: http://www.fgcu.edu/admissions.asp.

All other out-of-state students (those from any state not listed above) interested in pursuing an online degree/certificate program entirely from within their state of residency may not be eligible to apply (due to their home state’s law) and are invited to consult their state’s higher education licensing authority to discuss their situation: http://www.sheeo.org/stateauth/stateauth-home.htm.

IMMUNIZATION FORM
Please send this form or proper immunization documentation along with your application packet or directly to Student Health Services after you receive your University ID Number (UIN).  If you are a previous FGCU student, and/or currently enrolled at FGCU, these forms are already on file and don't need to be resubmitted.  Student Health Services contact information can be found on the immunization form above.

Age Exemption
If you were born on or before December 31, 1956, you do not have to provide immunization records, but you must call Student Health Services at 239-590-1254 to remove your immunization hold.

RESIDENCY FORM
**To receive Florida residency classification, applicants must complete, and submit this form with at least two supporting documents dating 12 months prior to the last day of registration.  Once term registration has closed, state law allows no change to residency status. 

SSN Statement:  Federal Law allows the university to require that you provide your U.S. Social Security Number for the purpose of identification and verification of student records, including, registration, financial aid, and academic records (such as GMAT, GRE, or MAT scores and transcripts), verification of identity in connection with the provision of university services, and for required state and federal reporting as well as institutional purposes.  By providing your SSN, you authorize the University to disclose it to third parties as necessary for these purposes.
