Congratulations on becoming an Employee at FGCU!

There are two requirements that must be met before you begin your job on campus.

Level 2 Criminal Background Check

All Non-Student OPS and Adjunct Faculty require a Level 2 Criminal Background Check which includes fingerprinting. Background Check results must be received and reviewed by HR before you may begin working. Fingerprinting is performed at the University Police Department by appointment.

    • Contact HR to obtain your New Hire VECHS waiver form and appointment instructions prior to attending sign-on or;
    • You will be provided with the New Hire VECHS waiver form and appointment instructions at the sign-on session

Conducted in the Modular 2 Conference Room

New employees must arrive promptly.
Those arriving late to a session will be required to attend a later session.

The following items are required to complete your Sign-On Session:

  • New Hire Background Check Verification Form*

Obtain the completed form from your Supervisor (NOT required for Adjunct Faculty). This form is required to participate in the sign-on session. If you do not have it on hand you will be asked to return to your Supervisor to obtain it and attend a later session.

*Please do not confuse this form with the VECHS Waiver form, which is required by UPD for ALL employees being fingerprinted.

  • Sign-On Packet

Print single-sided and complete in pen.

Important Note: A critical component of the new employee sign-on process is the completion of the Bureau of Citizenship and Immigration Services (BCIS, formerly INS) I-9 Form. Completion of the I-9 form is required to verify the identity and employment eligibility of anyone hired by the University. According to BCIS requirements, all new employees must complete the I-9 Form on or before their first day of work.

Social security card for Payroll W-4 purposes (photocopy is acceptable if you will not be presenting your original security card as one of the I-9 documents).
