What to Do if an Injury Occurs
Responsibilities of the supervisor
In a medical emergency, transport the injured employee to the nearest medical facility or call 911 for emergency assistance. Following the arrangement of emergency medical treatment for the injured employee, call
1-866-786-3351 to report the injury to CorVel Corporation.
If an injury requires non-emergency medical treatment, call 1-866-786-3351 immediately and prior to obtaining medical treatment. CorVel will assist you and the injured employee in selecting an appropriate medical provider from within the CorCare Network and will arrange the appointment. CorVel provides this service 24 hours a day/7 days a week.
After medical treatment is arranged, CorVel's First Response Unit (FRU) will provide the Division of Risk Management, the Workers' Compensation Coordinator, and the injured employee with a copy of the First Report of Injury (Fom DWC-1).
As supervisor, you should evaluate the circumstances of the occurrence and correct any unsafe conditions.
Responsibilities of the injured employee
Prior to an injury
Hopefully you will not experience a work-related injury, but you need to familiarize yourself now with this information in case a work-related injury does occur to you or a fellow employee.
If you are not familiar with your managed care workers' compensation program, please contact CorVel toll-free at 1-866-786-3351 and a CorVel representative will assist you in understanding the procedures and your responsibilities within the CorVel managed care program.
You need to familiarize yourself with the CorCare Network Provider Directory, which is available in Human Resources.
When an injury occurs
In a medical emergency, go to the nearest medical facility.
When you have a minor injury, report to your supervisor to document the injury. Receive necessary first aid at job site if available.
If the Injury requires non-emergency medical attention
- Immediately report the injury to your supervisor. Either you or your supervisor should immediately call 1-866-786-3351. A CorVel representative will promptly assist you in selecting a primary care provider from the CorCare Network ProviderDirectory and will arrange the medical appointment for you. Remember the choice of primary care provider is yours and CorVel is simply coordinating your medical treatment to assist you in receiving prompt and appropriate medical care.
- You must seek treatment from a CorVel medical provider listed in the CorCare Network Provider Directory for your service area. Failure to obtain medical treatment from a CorCare Network participating provider will jeopardize your workers' compensation benefits.
- Should you go directly to the Primary Care Physician without notifying the CorVel medical case manager, upon your arrival identify yourself as a member of the CorVel State of Florida Workers' Compensation Program.
- Follow medical treatment plan prescribed by your provider.
After Your Initial Medical Treatment
- Following the reporting of your injury to CorVel at 1-866-786-3351, you will receive an Employee Handbook in the mail from CorVel and the most current CorCare Network Provider Directory for your service area. A cover letter will inform you of the CorVel medical case management office assisting you with your injury.
- Please contact CorVel any time you have questions regarding medical treatment for your injury. This mailed Employee Handbook will contain a treatment access card with the CorVel medical case management office toll-free number. This is the card you will present to all your subsequent medical providers.
*Note: This toll-free number for the CorVel Medical Case Management office is only for arranging treatment for your present injury. Any new injuries must be reported to the 1-866-786-3351 toll-free number, not to the CorVel Medical Case Management office handling your current injury.
- You must contact the CorVel medical case manager assigned to your injury at the toll-free number on the new treatment access card for any future medical treatment to obtain the appropriate medical services allowable under your CorVel managed care program.
- During your medical treatment, if you wish a medical specialist other than the one chosen by your primary care provider, CorVel will allow for one referral to a similar specialist within the CorCare Network Provider Directory. All referrals or changes must be coordinated by your primary care physician and the CorVel medical case manager assigned to coordinate your medical treatment.
Pharmacy Benefits
- You are entitled to pharmacy benefits under your CorVel managed care program. Under the current Florida Workers' Compensation Statutes, you are entitled to go to the pharmacist of your choice.
- The CorCare Network Provider Directory contains a county listing of participating pharmacies. However, this should in no way be construed as limiting your choice of pharmacy providers to only those listed.
- If you use a pharmacy not listed in the CorCare Network Provider Directory or have not reported your injury to CorVel, then you will need to be prepared to pay for your prescription at the time of purchase. Simply submit the prescription receipt to CorVel Medical Claims Center (see address below) for reimbursement of all work-related injury prescriptions. A pharmacy not listed in your directory may, at their discretion, desire to bill CorVel directly for your prescriptions and CorVel will pay them directly, in accordance with Florida Statutes.
- Whenever appropriate, CorVel may, as outlined within Florida Workers' Compensation Statutes, recommend the substitution of generic brand prescriptions.

Workers' Compensation Prescription Drug Program
- CorVel offers a comprehensive prescription drug program. A pharmacy card is mailed to the injured employee upon notice of a claim and network pharmacies also receive electronic notification.

Changing your Primary Care Physician
If at any time you are dissatisfied with your Primary Care Physician, you may initiate a change by calling the CorVel medical case manager. Once a request is made, the medical case manager will assist you in selecting a new primary care physician within the network.
Second opinions
Either you or the Division of Risk Management may request a second opinion at any time regarding your medical care or treatment plan. To arrange for a second opinion you must contact the CorVel medical case manager. The medical case manager will arrange and notify you of your appointment.
Independent Medical Examination (IME)
Should you or the Division of Risk Management wish for an independent medical exam to be performed due to a dispute concerning medical benefits, compensability or disability, you must contact the CorVel medical case manager immediately (either verbally or in writing). The medical case manager will coordinate the independent medical exam.
