MEDICAL INSURANCE COMPLIANCE DEADLINE

Any student wishing to provide Alternative Insurance must complete
the Medical Insurance Compliance and return to International Services

DEADLINE FOR FALL SEMESTER IS AUGUST 1
DEADLINE FOR SPRING/SUMMER SEMESTER IS DECEMBER 1

• F-1 Students, please check your I-20 expiration date. If you need additional time to complete your current program of study, it is important that you do a Program Extension, by submitting new Financial Statement documents and the Recommendation for Extension of Program form to ISO, at least two months prior to the expiration date on your I-20.
• If you are Traveling Outside the U.S., it is important that your documents (passport, visa and I-20/DS-2019) are up to date and your I-20/DS-2019 form has a fresh travel endorsement (signature) provided by a DSO/RO within the last six months?

To Extend your Program of Studies at FGCU, you must provide to ISO the following three documents by the deadlines of April 1 or November 15.

Academic Advisor Approval: Contact your academic advisor to complete the "Recommendation for Extension of Program" form and send it to us.

Updated Financial Statement: See financial statement template. Submit Financial Statement. Follow the information on the Financial Statement according to the International Student Admissions Guide in Pages 8-10. If you need to extend the program (I-20) only for one semester, you may provide half of required amount indicated on the table of expenses depending on your level of stydies (undergraduate of graduate).

NOTE: ISO APPROVED MEDICAL INSURANCE COVERAGE THROUGHOUT THE ENTIRE CALENDAR YEAR IS MANDATORY. NO INSURANCE=NO ENROLLMENT!
