School Registration Form and Online Student Registration. Please download the School Registration Form.
Please download and fill in the School Registration Form, and return it as an e-mail attachment to email@example.com and firstname.lastname@example.org. You may include a copy of the School Registration Form with your check. Registration form must be submitted by email no later than Wednesday, November 25th, 2015. Registration fees must be postmarked by Saturday, December 5th, 2015. Please go to the Online Student Registration to enter all your students. You may continue to add students until the online registration closes. You may enter more students than you will actually bring. Online Student Registration must be completed by Friday, December 11th, 2015.
The registration fee is $12.00 per student, which covers the cost of a tee shirt designed for the event. Please make a check payable to "FGCU MATH CONTEST" for $12.00 times the total number of participating students and send to:
