| Provision of Survey Forms The Office of Planning and Institutional Performance (OPIP), determines which courses will require administration of the survey according to the guidelines provided by the state. Subsequently, the Office of Planning and Institutional Performance will provide the survey packets for each course requiring paper administration. College Responsibility Each college is responsible for the distribution of survey packets. The college distributes the packets to the faculty during the designated time period. Faculty Responsibility (Faculty Instructions) Each faculty member selects a student in the class to administer the survey. The faculty member absents him/herself from the class until the students complete the survey. The survey is administered at the beginning of the class, takes approximately 15 minutes to complete. Student Responsibility The student selected to administer the survey distributes the survey to each student in the class, reads the instructions on how to complete the survey, collects the completed surveys, removes the blank forms, seals completed forms in the envelope provided, and returns them to the Library Circulation Desk. For off-campus courses, the student gives the sealed envelope to the faculty member, who returns it to the designated office. |
