FGCU SAI: INSTRUCTIONS FOR STUDENT SURVEY ADMINISTRATOR



At the conclusion of each academic term, students are invited to evaluate their courses and instructors. Evaluation results are not seen by instructors until after grades are submitted for the term. Please note that each person’s evaluation form is anonymous.

Please follow these procedures in administering the survey:

    1. After the instructor leaves the room read the instructions provided to the class and distribute one survey to each student in the class.
    2. Instruct the students to complete the survey using a No. 2 pencil, or a blue or black pen.
    3. Allot approximately 15 minutes for the entire evaluation process.
    4. Collect the completed evaluations
    5. Remove blank forms
    6. Seal completed evaluation in the envelope provided.
    7. Return the evaluations to a the Library Circulation Desk.
