Information Technology Services (ITS) offers services to the FGCU community to support the needs of the University. Faculty, staff and students wishing to request core ITS services should use the online forms below. This collection of forms is created to gather necessary information specific to the service being requested to provide a more timely response.

All Access Request should be submitted by a department’s designated Authorized Account Requestor (AAR)
The "Access Request" form is intended to be used for the following purposes:
1. Adding or removing an employee to an existing resource
2. Adding or removing multiple employees to an existing resource; select the "Multiple accounts" option.New Resource Request
New Resource Request: Shared mailbox, Shared folder should be submitted by a department’s assigned Authorized Account Requestor (AAR)
The "New Resource Request" form is intended to be used for the following purposes:
1. Requesting Information Technology Services (ITS) to create a new security group, distribution list, or shared mailbox.

Secure PC ensures that computers stay up to date and secure by centrally managing installed software applications. This form will allow any faculty member to request an exemption from Secure PC.

This form is used to request any type IT Move, ranging from workstations moving within the same department or office, to a series of moves involving multiple departments and buildings.Network Printer Setup
